The Eye Research Sight Award Application for Recognition

The Eye Research Sight Award is a recognition award which can be given to any one or more individuals or an organization who has contributed or displayed outstanding and dedicated service to their Club or community with ideals of eye research in mind. The criteria may include fundraising activities, dedicated work on projects for the blind, or overall outstanding service for the goals of eye research.

This award must be given by the candidate’s peers. It cannot be purchased by an individual themselves. Lions, Lioness, and LEO Clubs are limited to one award per year. Other organizations or non-Lions may also apply for this award at any time.

The “Eye Research Sight Award” pin and a distinctive plaque will be given to this outstanding candidate. The Award should be presented at a special occasion such as a Mid-Winter Conference, a District Advisory meeting, or any other special function.

A narrative describing the candidate's work and dedication should be included below (1000 character limit. If you need more space please email narrative to lionroth4@verizon.net). Please submit this application at least sixty (60) days prior to the award date to guarantee delivery. Mass Lions Eye Research Fund Inc. Executive Board has the final approval for all candidates.

Commitment

Lions, Lioness or Leo Clubs may qualify for a Sight Award when there has been a minimum increase of $1,000 in their Eye Research donations from the previous year.  

A three year average may also be used at the discretion of the Executive Board. For all other applicants, a $1,000 check should accompany this application made out to Massachusetts Lions Eye Research Fund, Inc.
